to be completed. However, we will review other
applications as long as the information is current.
The application
must be signed and dated.
To ensure the
accuracy of our records, we may require an updated application
periodically.
E & O Coverage
Provide a copy of
the TPA’s current E & O
License
Provide a copy of the TPA’s home state license, certificate or
registration as required by the state.

Sold Business

Provide a current
copy of the TPA’s home state license, if expired.
Provide a current
copy of the TPA’s E & O coverage, if expired.
Provide a copy of
the TPA license for the group's headquartered state as required by
