Appointments  

Producer Appointments are a requirement of BPI.  Appointment paperwork will be provided as needed.

Required for Appointment 

            Disclosure Statement

             BPI will provide form as needed. 

              License                       

              Provide a copy of the their resident license and
              non-residence license for each state.

              or, if an appointment is required to issue a
              license

              Provide the completed application, along with the
              application fee, if applicable.

              Once the appointment is complete the application
              fee and appointment will be forwarded directly
              to the insurance department.
